Nasal Flu Consent Form

Students in years 7 to 11 inclusive are eligible to receive a nasal flu immunisation this year if they wish. The form below has been emailed to all parents, but there is still time to return the form for your child if you have not yet done so. Students in years 7 - 9 will receive their immunisation next Monday (18th October), and students in years 10 and 11 will receive their immunisation on Tuesday 16th November.

If you would like your child to have the immunisation and have not yet completed and returned the form, please print the form below and return it to Reception or email it to vlowes@hitchinboys.co.uk as soon as possible.
